The problem isn't appointments

Year-end QOF pressure usually gets diagnosed as a capacity problem. Not enough clinics, not enough nurse time. In our experience the capacity often existed earlier in the year. What was missing was knowing who to put in it.

Getting one patient to a review that counts requires someone to interpret the indicator rules correctly, cross-check who's already been seen and invited, get an invitation out through a channel that patient actually reads, notice when it doesn't land, try again differently, and make sure the review gets coded so the indicator recognises it. Across every indicator and every register, that chain usually sits with one or two people who have a great deal else to do.

The work isn't hard. It's continuous, and continuous is what a practice can't spare a person for.

What compression costs you

Late invitations perform worse. A patient contacted three months ahead can book around their work; the same patient offered a slot in nine days declines, so you invite again, and your cost per completed review rises while attendance falls.

It also spills outward. Clinics filled with chased reviews in February aren't available for the patients ringing up that week, so a QOF problem quietly becomes an access problem in a different part of the practice.

And the scramble favours the reachable. The patients who answer the phone get seen; the ones who need the review most are often the hardest to contact, and they're the ones dropped when the deadline arrives. The threshold still gets met. The same people miss out every year.
